Social Services Care Navigator

Location: New York, NY

The Social Services Care Navigator provides information and guidance for callers trying to navigate care services for themselves or their family members. Population being served is typically elderly so background in Medicare and Medicaid and health insurance alternatives is important.

Job Duties:

  • Remains logged into a phone system, ready to take incoming call inquiries for scheduled shift.
  • Answers requests timely with courtesy and compassion.
  • Provides optimal customer service by being knowledgeable about available community services and insurance products.
  • Inputs call information accurately and completely into computer system.
  • Follows company and client training guidelines to answer questions, forward caller to appropriate area, and order literature for the caller.
  • Interfaces with client in accordance with company and client guidelines.
  • Assists with satisfaction survey calls, report updating, and special projects as needed.

Location: Virtual – Home Based, Must reside within the New York City Area

Operating Hours: Saturday and Sunday between the hours of 8am to 8pm, minimum 10 hours per week

Job Requirements:

  • High school diploma required.
  • 1-3 years of experience in social work required or equivalent military experience.
  • 1–5 years of customer service experience, ideally in a healthcare setting, health insurance or physician office.
  • Knowledge of Medicaid, Medicare and medical terminology required.
  • Confident, respectable and warm telephone disposition.
  • Spanish fluency desirable.
  • Excellent command of the English language, telephone etiquette, and strong communication skills.
  • Experience working with and communicating with the Elder population.
  • Ability to navigate within the Windows operating system.
  • Proficient performing web searches.
  • Must have demonstrated excellent interpersonal, and strong team orientation.
  • Accurate data entry skills.
  • Flexibility with work hours.
